What You Will Do:
The Death Registry Clerk is responsible for initiating death certificates and supporting vital records functions related to death and birth documentation. This role ensures timely and accurate processing of death data, supports compliance with state reporting guidelines, and provides administrative support in related vital record areas.
While the primary focus is death certificate initiation, the position also includes support tasks tied to birth records, follow-up documentation, and communication with internal and external stakeholders.
Death Certificate Duties:
Initiate death certificates in the electronic vital records system (e.g., EVERS).
Review medical records to assign appropriate certifying physician and forward to the funeral home.
Identify cases that require coroner review and assign to the correct county (excluding Jefferson County, which initiates their own).
Follow up with Decedent Affairs to ensure all records are entered, and notify them of physicians needing to register in EVERS.
Keep a detailed spreadsheet of missing death records, including documented reasons for each.
Collect data on pregnancy-associated deaths and request corresponding medical records for further review.
Birth Records and Follow-up Support:
Review birth or prenatal records for high-risk birth edits.
Assist with data accuracy in birth records, including verifying legal baby names and tracking Acknowledgment of Paternity (AOP) documentation.
Receive returned AOPs via mail and forward them to the Birth Registrar for correction.
Print and mail follow-up letters and forward any returned mail to the follow-up team.
Additional Duties:
Perform other duties as assigned to support the registry and vital records functions.
Serve as a backup for the Birth Registry team as needed.
Must obtain Notary Public certification within 30 days of hire.
What You Will Need:
High school diploma or equivalent required.
1-3 years prior experience in healthcare, medical records, or a vital records environment preferred.
Familiarity with electronic vital records systems (EVERS or similar) is preferred.
Ability to maintain confidentiality and adhere to HIPAA and state guidelines.
Notary commission (or willingness to obtain within 30 days of hire).
