What You Will Do:
The Tumor Registry Follow-up HIM Tech II will process, analyze, and ensure all medical records are complete within the required time frame. Ensure the integrity of the electronic medical records through data analysis, review, and reporting. Provide a timely response for requests of medical information according to departmental and hospital release of information guidelines.

Job Description/Responsibilities:

  • Ensure the integrity of the electronic medical record through analysis and review. Interacts and communicates with medical and professional staff to assist with research, audits, and timely completion of medical records.

  • Responds to requests for release of medical information for direct patient care from internal and external parties according to departmental and hospital guidelines.

  • Coordinates completion of patient’s medical record and ensures appropriate chart information is present and takes appropriate steps to secure any missing information.

  • Works with supervisor and/or manager to identify and solve problems, decrease any workflow restrictions, implement process improvements, and ensure timely completion of data collection.

  • Perform other duties as assigned.

  • Review clinical documentation as appropriate. 

  • Collect follow-up data on all cases of malignant disease.

  • Responsible for updating and maintaining cancer registry data.

  • Extract data and apply ICDO-O codes.

  • Participate in research, education, and monitoring for quality improvement activities.

  • Ensure data integrity and compliance with the American College of Surgeons Guidelines

  • Translate medical terminology into standardized codes to capture patient information on the quality of life for cancer patients and length of survival.

  • Performs follow-up using available resources such as phone calls or internet searches to identify the vital status of a patient (available resources: Social Security Death Index, Obituaries, Cerner, other hospital EMRs, tributes.com, etc.)

  • Retrieves information from outside facilities, MD offices, or registries as requested for completion of abstracts or follow-up.

  • Helps with data management projects, data analysis, and reports as needed by registrars and/or leadership.

  • Code and enter data into the registry database utilizing:

  • ICD-O, ICD-10-CM, AJCC TNM (Tumor, Nodes, Metastasis), Site-Specific Data Items (SSDI), STORE (Standards for Oncology Registry Entry), and SEER (Surveillance of Epidemiology and End Results) guidelines

  • Complies with state and federal mandates that require reporting all diagnosed and /or treated malignancies and reportable benign tumors.

  • Perform data quality control activities on registry data.

  • Contact patient providers and state registries per the database to gather follow-up information and verify or correct patient information.

  • Maintain the highest degree of confidentiality of all information encountered including verbal, written, and computerized.

  • Reports to the manager any failure by anyone to protect confidential information.

  • May require working overtime as the work permits.

  • Requires a consistent work schedule.

What You Will Need:

  • Associate’s degree

  • Certified or eligible to sit for the national exam for Registered Health Information Technology (RHIT) by the American Health Information Management Association (AHIMA)

  • If RHIT eligible, certification must be obtained within six (6) months from the date of hire.

  • 5 years of experience working in HIM

The annual salary range for this position is $49,400.00-$74,200.00. Compensation decisions depend on a wide range of factors, including but not limited to skill sets, experience and training, security clearances, licensure and certifications, and other business and organizational needs.
